CONSENT FORM:

At the time of admission you or your next kin or a responsible person bringing you for admission will have to sign a consent form.

Before surgery, or any procedure that requires consent form the patient, you will be asked to sign one.
Your treating consultant or the doctor performing the procedures will explain you in detail the requirement, nature, options available, advantages, disadvantages if any and the risks involved with the procedure.

If you need to have some clarification, you should ask the doctors involved and get yourselves satisfied. It is advisable to sign the consent only after all the doubts are cleared.

For patients less than 18 years of age, consent form must be signed by parents or guardian.
